Ctrl+Left Click on a shortcut does not open a link in a new tab. I assume shortcuts are probably buttons rather than links, but typical key combinations are useful.
The background photo taking a quarter second to fade in is annoying. Having the extension ready all the time in the background would be nice, that way the startpage only has to be loaded on firefox start.

You can open link in a new tab by clicking with the mouse wheel, but you're right, we'll add the ctrl+click combination too.
As for the background taking time to load, new tab pages are just website pages and can only load when you request it. You can add this line to your custom style settings to see if you prefer without fade in:
#interface #background_overlay { transition: none }
