Restores the toolbar and add-on icon, which was missing in 1.0.1.

The main fix: when you run out of time, the block is now a fixed cooldown. Before, it only lifted once you'd re-banked several minutes of budget, which at the default rate took around half an hour, so it looked like your time never came back. Now you set how long the block lasts, your budget refills the whole time, and the popup shows the balance climbing with a countdown to when YouTube unblocks.

Also in this version:
- The daily limit is set in hours per day (your max watch time per day) instead of seconds per minute.
- Tightening a rule saves right away; only loosening one still needs the confirmation step.
- New defaults: 3 hours a day, 10-minute cooldown.
- Cleaner popup and settings layout, with status and app icons.
