I can't replicate this issue. Could you send me your settings file (Options page > Import / Export > Export settings to file)?

The background property of .ytp-tooltip-bg element is an URL to the thumbnail image. You can check what exactly is blocking it in DevTools > Network tab (while you're hovering over the progress bar).
